Care Coordinator, Ardleigh
Ardleigh Surgery
Ardleigh Surgery has an exciting opportunity for a Care Coordinator to join our practice. The role is offered on a full-time basis and involves working very closely with the practice team and is pivotal in ensuring all patients receive the best possible care and service.
Ardleigh Surgery is situated in the semi-rural town of Ardleigh, with a branch surgery in the picturesque constable country village of Dedham. They have a list size of around 7,500 patients. The team includes 5 GP’s, a Nursing team of 8, a Dispensing team as well as the practice manager, administrative and reception staff.
The successful applicant will:
Ensure a high standard of service delivery.
Be a key point of contact for both patients and carers, supporting their journey through primary care.
Support the practice in coordinating all key activity including access to services and advice.
Book appointments and run searches to identify vulnerable patient groups.
Assist in running the local Dementia café.
Manage the populations health initiatives and campaigns.
To be considered for this position, applicants should have the following:
Excellent interpersonal communication skills.
Be able to speak professionally and confidently to different patient groups, colleagues and clinicians.
Understand primary care or have been in an NHS setting in the past.
Be proactive and able to work independently, using their own initiative, but able to take and respond to detailed instruction where required.
Have excellent record keeping and IT skills.
Be able to prioritise multiple tasks and achieve them all to a high standard.
Previous experience in a healthcare setting is desirable but not essential.
