Social Prescriber, Feering
The Colte Partnership
The Colte Partnership has an exciting opportunity for a Social Prescriber to join our team on a part-time basis for 25 hours per week. This role will play a vital role in enhancing patient care and well-being by connecting individuals with non-medical support services within the community.
The Colte Partnership provides primary care to around 80,000 patients across north-east Essex from 8 GP practices., with our head office based in Feering. While our dedicated branch teams continue to deliver excellent clinical care to our patients, our executive team, focus on our longer-term ambitious vision of the partnership and creating a sustainable primary care service long into the future.
The post holder’s responsibilities will include:
Collaborate with healthcare professionals to identify patients who would benefit from social prescribing.
Conduct holistic assessments to understand patients' social needs and preferences.
Develop personalised care plans and signpost patients to relevant community resources and services.
Educate patients on available social support options and empower them to take control of their health.
Organise and facilitate group activities that promote mental well-being, physical health, and social inclusion.
Maintain accurate records of patient interactions and outcomes in accordance with data protection regulations.
There is a wide socio-economic and demographic served within this area which will provide an interesting and varied case mix. You will be joining an enthusiastic team of clinicians and administrators. You will be well supported with on-going professional development (training support, CPD and peer support). The role will contribute to the improving quality of care of our patients across the network of practices.
Preferred experience:
Experience in promoting wellbeing services to facilitate independence personal resilience.
Experience in creating wellbeing assessments that take into consideration the role and possible impact of the wider determinants of health on wellbeing.
Experience in building relationships with key staff in GP practices , attending relevant meetings, becoming part of the wider network team, giving information and feedback on the role of social prescribing.
